Description of Position:

The C.A.R.E. Program provides treatment, rehabilitation, and support services for individuals with severe and persistent mental illness. The multi-disciplinary staff on the C.A.R.E. team work together to provide services which enable individuals with severe and persistent mental illness to live and participate successfully in their communities. The CARE Team Member provides direct residential services to individuals with severe and persistent mental illness. Hours for this position are 2pm to 10pm and every other weekend.

Job Knowledge and Comprehension:

• Provides specialized treatment, assistance, education, rehabilitation, and support services as part of a comprehensive team to C.A.R.E. team/transitional residence clients.
• Participates as part of a comprehensive team in the treatment planning process and monitors client progress in meeting the goals and objective of the treatment plan.
• Assists C.A.R.E. team/transitional residence clients in improving their social and residential adjustment and in developing and strengthening of daily living skills.
• Assists and supervises C.A.R.E. team/transitional residence clients completing daily household chores, meal preparation, and other tasks.
• Maintains records of medication and other unusual incidents in accordance with applicable regulations.

Minimum Qualifications:

• High School Diploma or GED required
• Associates degree in Social Services or Human Services or some combination of education and experience preferred.
