Berkley Home Health Care a locally owned, award-winning, Colorados' Home Care leader and member of 2011-2021 Home Care Elite, a 5 Star CMS rated agency, Top 500 Agency has an immediate part-time position for an experienced Home Health Occupational Therapist to join our team in the Denver, Aurora, Littleton, Highlands
Ranch, Lone Tree, Centennial areas.

Occupational Therapist Duties include:

• Initiating, evaluating, developing, implementing, documenting, supervising, and revising plans of care for patients requiring occupational therapy.
• Coordinating patient care with other staff members and physicians.
• Documenting patient assessment, needs, and care
• Assisting with coordinating and teaching rehabilitative care programs to meet the needs of staff, patients, caregivers, and the community

This job requires a person who can work independently, while never forgetting that home care drives patient success through the incredible effort of our amazing team.

Other requirements include:

• Graduate of an accredited Occupational Therapy program or American Occupational Therapist Association Certification
• Licensure by the State of Colorado
• Knowledge of geriatric OT
• Ability to communicate effectively, both verbally and in writing, Respecting the patient’s privacy and maintaining the confidentiality of patient info
• Current Colorado driver’s license and automobile liability insurance
• CPR Certification

Pay: $50.00 - $70.00 per hour
