We are a local closed door Hospice Pharmacy looking for a Pharmacist who is able to fulfill the following responsibilities:

Position:

• This is a Full Time Position (Salary) that would include some rotating weekends and on call shifts.
• On-Call Pharmacist may work from home to answer calls HOWEVER must be available to go into the pharmacy to fulfill emergency orders if needed.
• Provide clinical consultative services to patients and therapeutic recommendations to nurses, physicians, and other hospice personnel.
• Enter, access, and retrieve data in the pharmacy system.
• Transcribe, verify, and fill prescription orders.
• Verify and fill medications accurately and efficiently.
• Majority of On-call orders or calls come in before 11pm.

Preferred:

• Exceptionally strong written and verbal communication skills, with special emphasis on effective telephone correspondence.
• Excellent problem solving skills, with a demonstrated ability to prioritize.
• Ability to work with minimal supervision and maintain courteous and professional composure during stressful situations.

Required:

- Pharmacist License that is current and in good standing with the State Board.

- Lives within 15-20 minutes of the Pharmacy located in Chatsworth (THIS IS A MUST)
