As a Physical Therapist, you are primarily responsible for the Product of having fully documented and highly satisfied patients that complete their full treatment program.

A Physical Therapist, you will primarily be responsible for the statistics of Patient Visits per Hour, Planned Discharge Percentage, and Average Number of Patient Visits to Discharge.

• Delivers competent, customized, hands-on, evidence-based, and compassionate care to clients.
• Continually seeks to better clinical skills through continuing education opportunities, in order to provide our patients with the highest quality of physical therapy available.
• Strives to ensure patients are completing their treatment plans and achieving maximal results with therapy.
• Routinely uses functional measures in evaluating the effectiveness of the treatments being administered.
• Completes and submits daily treatment notes in a timely manner and accurately charges for the services being provided in our facility.
• Seeks opportunities to build rapport and relationships with referral sources within the community.
• Maintains confidentiality regarding private client, employee, and organizational operations information.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Evaluates patients with a variety of diagnoses and creates customized treatment plans based on evidence-based practices to effectively enable them to achieve their goals.
• Ensures the care given meets a high clinical standard of excellence according to what research shows is “best practice.”
• Outlines clear functional goals for patients and the expected outcomes of the treatment plans.
• Educates patients and/or family members about the recovery process and plan of care.
• Regularly monitors patient progress and adjusts the plan of care as needed.
• Thoroughly documents and submits evaluations and daily treatment notes in a timely manner and accurately charges for the services being provided in our facility.
• Regularly updates documentation and maintains current prescriptions, authorizations, and signed plans of care for treatment.
• Enhances professional growth and development through participation in educational programs, in-service meetings, and workshops.
• Seeks opportunities to build rapport and relationships with referral sources within the community.
• Stays current with the latest evidence-based research in our field.
• Provides direction and leadership to our staff and acts as a resource to promote physical therapy within our community.

Shift Details:

Flexible Schedule - 4, 10-hour days, or 5, 8-hour days. We are open to scheduling around a therapist's personal preference on shift (i.e. we're flexible if they want to work from 7-4, 8-5, 9-6, or a combination of those on different days).
