We are accepting applications for a self-motivated, Social Service Director for our long-term care facility. Benefits package including Health, Dental, and vision insurance, Short term disability and life insurance.

Job Description:

• Conducts interviews with residents, families and/or staff for purposes of MDS completion. Documents in the electronic medical records.

• Develops/implement Patient Care Plans. Completes care plan in the electronic medical record.

• Conducts required/needed care conference meeting as needed

• Interprets the care plan to residents and families.

• Records progress notes in electronic medical record concerning services rendered in relation to a residents personal and social problems and/or needs.

• Assists in discharge planning. Coordinates community-based services and/or transfer requirements to another facility.

• Interprets the psycho-social functioning of residents to the interdisciplinary care team. Provides emotional support to residents and families.

• Utilizes community resources where appropriate.

• Coordinates and participates in hospice evaluations and enrollments.

• Coordinates and assists with resident room transfers.

Requirements:

• Education: Bachelor’s degree in Social Work or a related field (Masters Preferred).

• Long-term experience minimum 2 years

• Skills: Thorough knowledge of the case work process and care planning.

• Familiar with Long Term Care/SNF regulations
