We are seeking patient focused, self-motivated, reliable and independent staff members to join our team. This position requires someone who is detail oriented, flexible, adjusts easily to a changing work environment and can work independently with a focus on excellent patient care. The position is full time with weekends and holidays off. Dialysis experience is not necessary!

Duties:

• Patient care - Identifies patient care requirements and needs by establishing personal rapport with potential and actual patients and other persons in a position to understand care requirements.
• Teaching - Establishes a compassionate learning environment by providing emotional, psychological, and spiritual support to patients and families. Promotes patient's independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; answering questions.
• Quality care - Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following hospital and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.
• Case management - each full time nurse is responsible for care and oversight of up to 20 patients. Once training is completed, the nurse monitors the patient health status through monthly visits, phone calls and electronic treatment review.
• Interdisciplinary team – participate in a dynamic team that includes the physician, dietitian and social worker to provide exceptional patient care. Resolves patient problems and needs by utilizing multidisciplinary team strategies
• Autonomy – as you provide care in our independent setting you will use your nursing skills to assess, triage and implement interventions by looking at the whole patient.
• Safety - Maintains safe and clean working environment by complying with procedures, rules, and regulations. Protects patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures.
• Professionalism - Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.
• Team environment - Maintains a cooperative relationship among health care teams by communicating information; responding to requests; building rapport; participating in team problem-solving methods.

Requirements:

• Current Registered Nurse (RN) license
• One year of nursing experience (in any field)
• Associates Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from accredited school of nursing required; three-year diploma from accredited diploma program may be substituted for nursing degree
• Basic computer skills and proficiency in Microsoft Word, Excel and Outlook
• Ability to lift up to 30lbs
• Previous experience in dialysis is helpful but is not required
• Bilingual in Spanish is a plus

Salary Range: $40 to $54 per hour depending on experience with a comprehensive benefit package.
