Under the administration supervision of the Program Assistant and is clinically accountable to the Chief of Social Work Service. The incumbents will be responsible for the following duties but not limited to:

• Conduct or co-lead weekly skills building groups
• Meet with and provide social work services for patients who have a need for direct social work services as defined by the Social Work Service
• Develop and maintain contact with patients’ families as appropriate
• Participate in all 7-day, 30-day, 60-day and 90-day reviews of each patient on the unit, provide input on progress in treatment
• Report on family, social and community resources
• Identify further treatment needs and make recommendations
• Prepare 90-day treatment plans that reflect patients’ current and ongoing treatment needs, including dictation of the narrative summary for patients on assigned caseload
• Write social work progress notes describing and evaluating the patients’ responses to social work services received
• Collaborate with the Interdisciplinary Team, State’s Conditional Release Program, Department of Corrections and/or Parole Agents in formulating an outpatient treatment contract/discharge plan and facilitate the patient discharge to the dispositional setting

• 10 hour shift
• 8 hour shift
• Monday to Friday
